Supports Community nursing
NDIS Community Nursing at Home in the Sunshine Coast
Nursing care in your own home, coordinated alongside your everyday supports so the people helping you are working from one plan.

Clinical care that fits around the rest of your support
Some participants need nursing care alongside their daily supports: wound care, medication management, catheter and continence care, or the oversight of a complex health condition. Under the NDIS this is funded where it relates to your disability, and it has to be delivered by a registered nurse.
What goes wrong most often is not the nursing. It is that the nurse, the support workers and the family are working from different information. We coordinate the two so that the worker who is there every day knows what the nurse has put in place.
Ask us about your specific needs. We will tell you what we can arrange, what we deliver directly, and where a specialist nursing provider is the better answer.

What community nursing can cover
Depends on your plan and your needs. These are the common ones.
Medication
Management, administration and the training of support workers where the plan allows.
Wound care
Dressings, monitoring and escalation when something changes.
Continence and catheter care
Handled properly by people trained to do it.
Complex health
Oversight of conditions that need a clinical eye alongside daily support.
Training your workers
High intensity supports need trained workers. A nurse trains and signs them off.
Coordination
Making sure the nurse, the workers and the family are working from the same page.

Questions about community nursing
Is community nursing funded by the NDIS?
Yes, where the nursing need relates to your disability and not to a general health need. Health system responsibilities still sit with Medicare and the hospital system. We help you understand which is which.
Who delivers the nursing?
A registered nurse. Support workers can be trained and signed off by a nurse to do specific high intensity tasks, but the clinical care itself is nursing.
Can my existing support worker do the nursing tasks?
Some tasks, once a nurse has trained and signed them off under a high intensity support plan. Others, no. We will tell you which is which.
What if my needs are more complex than you can meet?
We say so and help you find a specialist nursing provider. We would rather refer you than do it poorly.
Tell us what nursing care you need.
We will tell you what we can arrange.
