A person can have a housing worker, a support coordinator, a GP, a psychologist and three support workers, and still have nobody who knows the whole story. Every service holds one part of it. Nobody holds the shape.
Case management is the practitioner who does. Who was at the last appointment, what changed, what is due, and what everyone else has already tried.

The same practitioner, who does not need the history explained again each time something happens.

When something goes wrong at 4pm on a Friday, there is someone who already knows the situation.
Case management or support coordination?
They overlap, and the words get used loosely. Support coordination is funded to help you find and connect services. Case management is the practice work of holding a complex situation together once they are connected.
We do not provide support coordination. If you have a coordinator, we work alongside them and do not duplicate what they do. If you are not sure which you need, tell us the situation and we will say which it sounds like, including when the answer is neither.
Who this suits
People with several services involved at once, plans with a history of things being missed between reviews, and households where more than one system is asking for something at the same time.
It is not for everyone. Where a plan is straightforward and working, this is a cost without a job to do, and we will tell you that.
